Medical Forms for the 2024 Season

Part of the registration process is new Medical History Form. The Medical History Form is required to receive your football equipment on equipment handout day (to be scheduled later) and start practicing. This Medical History Form can be found here:https://www.mfjrphoenix.com/Default.aspx?tabid=1102688 or by going to our website and clicking on the 'Safety Info' tab then clicking 'Safety Forms'.

A couple things for the Medical History Form:
1. You must use the league form which is in the link above. General school forms, WIAA forms, and doctor forms will not be accepted.
2. The form must be signed off by your family doctor/health care facility.
3. The form must be dated with a date in the year 2024.
4. The form must be completed in its entirety.
5. The form is to be handed in on equipment pickup day. We need the physical form for our on-site safety crew in the event of an emergency.
